A strategy guide for Plague Inc: Evolved covering the core infection-lethality-cure balance, disease types, a reliable Normal/Brutal opening, the 2026 Aliens and Anti-Vaxxers DLC with the new Xenolith disease, multiplayer, Cure Mode and achievement tips.

Researched September 2026 — current as of v1.24 (World Update 2026, August 3, 2026) and hotfix 1.24.0.3 (August 10, 2026). Live games change; check the linked sources if something differs.

Quick Start

  • Infect first, kill later. The winning pattern for most diseases: spread silently to every country, then add lethal symptoms once cure research can't catch up.
  • Watch the cure bar. Humanity's cure research is your timer. Everything you do either speeds it up (being noticed, being deadly) or slows it down.
  • Severity gets you noticed. Symptoms raise severity; high severity makes countries research the cure sooner. Devolve symptoms that appear randomly if you're still spreading.
  • Pop every bubble. Orange DNA bubbles and red infection bubbles (which appear when a new country gets infected) both give DNA. Popping them is your main early income.
  • Get to hard-to-reach countries early. Island nations and cold/rich countries (e.g., Greenland, Iceland, New Zealand, Madagascar) can close ports and airports. Invest in transmissions that bypass travel.
  • Drug resistance and climate resistance matter. Rich countries have strong healthcare; cold and hot countries resist diseases without the matching ability.
  • Genes are free power. Pick genes before starting — they give early DNA, faster spread or cure resistance.
  • Difficulty changes the AI. On Brutal and Mega Brutal, humanity reacts faster, researches harder, and closes borders earlier.

How the Simulation Works

You spend DNA points to evolve three trait trees:

Tree | What it does

Transmission | How your pathogen spreads (air, water, blood, insects, animals, livestock, rodents, birds)

Symptoms | What it does to people; raises infectivity, severity and lethality

Abilities | Resistances (cold, heat, drugs) and genetic tools that slow the cure

Three stats define your plague:

  • Infectivity: how fast it spreads within and between countries
  • Severity: how noticeable it is; drives public concern and cure funding
  • Lethality: how fast it kills the infected

The core tension: lethality kills your hosts, and dead hosts don't spread. Kill too early and you burn out before reaching every country.

Cure research

Once your plague is noticed, countries contribute to a global cure. Rich countries with good infrastructure research fastest. You slow the cure by:

  • Staying unnoticed longer (low severity)
  • Buying genetic abilities that increase the cure's requirement
  • Killing or disrupting researching countries late in the game

Government actions

Countries respond with actions: closing airports and seaports, handwashing campaigns, martial law and more. These are why hard-to-reach countries must be infected early.

Disease Types

The base game advertises 10 disease types. Each plays differently:

Type | Core twist

Bacteria | Balanced; solid starter

Virus | Mutates symptoms randomly; fast but unstable

Fungus | Struggles to spread between countries; relies on spore bursts

Parasite | Low severity; stealthy

Prion | Slow and stealthy; manipulates the brain to slow cure research

Nano-Virus | Detected immediately; race against the cure

Bio-Weapon | Extremely lethal from the start; manage it carefully

Neurax Worm | Controls human minds; unlocked specially

Necroa Virus | Zombie virus; a combat-flavored mode against human forces

Simian Flu | Infects apes as well as humans; ape colonies spread it

Plus the Shadow Plague (a vampire disease type) and, new in 2026, Xenolith (the aliens disease type from the Aliens and Anti-Vaxxers DLC).

A Reliable Standard Opening (Bacteria, Normal to Brutal)

This general route works for most "silent spread" diseases:

  • Pick genes for early DNA and spread (see Advanced Tips).
  • Start in a densely populated, well-connected country. Countries with lots of land borders and travel links (e.g., in South Asia or the Middle East) spread quickly both locally and globally.
  • Early DNA into transmission: Air and Water level 1 early. Add a matching climate resistance based on where you want to go next.
  • Get Drug Resistance once rich countries matter.
  • Leave symptoms alone. Devolve anything that appears randomly while you're still spreading.
  • Once every country is infected (or close), evolve abilities that make the cure harder.
  • Then go lethal. Stack the heavy symptoms all at once so countries collapse before the cure finishes.
  • Track holdouts. If one isolated country isn't infected, invest in the transmission that reaches it (e.g., birds or high-level air for islands).

Adjustments by difficulty

  • Casual/Normal: You can be sloppier. Going lethal a bit early is fine.
  • Brutal: Countries close borders sooner. Get resistances and transmission earlier; delay symptoms longer.
  • Mega Brutal: The cure moves fast. Genetic abilities that slow cure research are essential, and your infection must be near-total before any lethal symptoms.

The 2026 DLC: Aliens and Anti-Vaxxers

Released April 7, 2026, this paid DLC (store title "Plague Inc: Aliens & Anti-Vaxxers") adds:

Xenolith (Aliens): crash a meteor into Earth and transform it into a crystalline world. Instead of DNA, Xenolith uses XNA, and much of its play revolves around crystals and xeno-forming. G-COM is the human response researching you. Patch notes show:

  • Humans deploy Meteor Defense technology when needed.
  • On Brutal and higher, multiple asteroid impacts damage the planet and reduce passive XNA gain.
  • On Brutal and higher, hoarding too much XNA reduces passive gain if Xenolith isn't big enough.
  • Patch notes mention Xenolith tools such as Frequency Shift (which can be used against humanity's Prototype Disruptors) and Metabolic Hijack.
  • Brutal and Mega Brutal were rebalanced shortly after launch (slightly less XNA, faster research, pricier abilities).

New scenarios and modifiers listed on the DLC store page:

Content | What it is

Zombies vs. Earth | An undead plague released from a melting glacier in Greenland

Anti-Vax | Spread weaponised conspiracies and misinformation as vaccine confidence collapses

Hell on Earth | A heat-loving plague that corrupts people, animals and the world itself

Herd Immunity (modifier) | Infected people can recover and gain natural immunity

Measles | Resurgence of a vaccine-preventable disease

Cure Mania (modifier) | Scientists share breakthroughs in real time and launch cure planes more often

Leprosy | A slow bacterial disease scenario

Early patches tweaked Leprosy starting DNA, trait costs, infectivity and score multiplier (easier 3 biohazards), and updated scoring and win requirements for Zombies vs Earth and Herd Immunity.

Xenolith tips

  • Don't hoard XNA on higher difficulties — spend it to grow, because oversaving reduces passive gain.
  • Watch for meteor defenses; they are humanity's answer to your impacts.
  • Use crystal alignments deliberately; patch 1.23.0.8 fixed the Lattice Shedding gene generating too many crystal seeds when whole continents aligned, so don't rely on that behavior.
  • G-COM researches you more easily if you sit on large amounts of unused XNA (1.23.0.7), another reason to keep spending.

The 2026 World Update (v1.24)

  • Country data now uses the latest World Bank datasets, raising the world population to 8,212,708,793.
  • The simulation update applies across the game, including Xenolith.
  • A free Plague Legacy scenario restores the 2012 world for classic play.
  • Fixes: Cure Research going negative in Xenolith, inaccessible Active Abilities, Xenolith Genes counted in stats, and the Darwin Award Outbreak modifier picking starting traits.
  • 1.24.0.3 fixed a crash starting the Peripheral Verge Outbreak.

Other Modes

  • Scenarios: The base game lists 23 unique scenarios (for example a new Swine Flu strain or an Ice Age world), with more added by DLC.
  • Custom scenarios: The content creator and Steam Workshop host over 10,000 community scenarios.
  • Competitive multiplayer: Two plagues fight for genetic dominance; players get new evolutions, abilities and genes.
  • Co-op: Two diseases team up while humanity fights back with new tricks; smash cure labs together.
  • Cure Mode (The Cure DLC): Play as humanity, trying to stop an outbreak.
  • Cheats: Unlocking cheats no longer requires completing Xenolith (patch 1.23.0.9).

Achievement and Biohazard Notes

  • Scenario achievements need Normal or higher. Patch 1.23.0.7 closed a bug that let scenario completion achievements unlock on Casual, so Casual clears no longer count.
  • "Get 3 biohazards with each disease" achievements (such as Mr. President) had their conditions improved in 1.23.0.11. If one didn't pop for you before April 2026, replay a clean 3-biohazard win.
  • Leprosy is now a friendlier 3-biohazard target: 1.23.0.8 raised its score multiplier and tweaked its starting DNA and trait costs.
  • Xenolith Genes count toward the "Unlocked Genes" stats since v1.24, so unlocking them now contributes to gene-collection progress.
  • Cheats no longer require a Xenolith clear (1.23.0.9); just remember cheat games are for fun, not achievement hunting.
  • Where is Everyone: a long-standing infection-rate bug in this low-density scenario was fixed in 1.23.0.10, so older strategy advice for it may be off.

Common Mistakes to Avoid

  • Evolving symptoms early. The number-one reason for losing on higher difficulties.
  • Ignoring isolated countries. Greenland and island nations close their borders; get them early.
  • Skipping drug resistance. Rich countries with strong healthcare will stall your spread.
  • Not devolving random mutations. Especially with Virus.
  • Going lethal before the world is infected. Burnout ends many runs.
  • Ignoring cure-slowing abilities. On Brutal+, you need them.

Advanced Tips

  • Genes: Early DNA genes such as ATP Boost (extra starting DNA) and Metabolic Jump accelerate openings, and cure-focused genes such as Genetic Mimic buy time late. Read each gene's tooltip for the current effect; more unlock as you win with each disease type.
  • Starting country: Choose based on the disease. Stealthy diseases like being far from wealthy research hubs; fast spreaders like dense, connected countries.
  • Bubble discipline: On early speeds, pause and pop every orange bubble. That DNA compounds.
  • Timing the switch to lethality: When the infected count is huge and the cure is nowhere near done, buy your deadliest symptoms in one burst.
  • Biohazard ratings: You earn up to three biohazards per win; higher difficulty and faster wins score better.

FAQ

What's the easiest disease for beginners? Bacteria. It's balanced and forgiving.

What's the best starting country? It depends on the disease, but densely populated, well-connected countries are a safe bet for most.

How do I beat Mega Brutal? Spread silently to every country, get resistances early, invest in cure-slowing genetics, and only then go lethal — all at once.

Is the new Xenolith disease free? No, it's part of the paid Plague Inc: Aliens and Anti-Vaxxers DLC (April 2026). The World Update 2026 itself is free.

What is Plague Legacy? A free scenario that returns the world to its 2012 state, for players who want the original data.

Does multiplayer still work? Yes. Recent patches (1.23.0.11) fixed multiplayer government actions and co-op country selection.
