How to build profitable, three-star hospitals in Two Point Hospital: layout, the patient pipeline, staff and training, money management, research, the star system and DLC overview. Includes the most common mistakes that sink new administrators.
Researched September 2026 — current as of the October 2025 security hotfix (CVE-2025-59489 patch); the game is feature-complete with 7 expansions. Live games change; check the linked sources if something differs.
Status: Two Point Hospital (Two Point Studios/SEGA, released 29 August 2018) is finished and no longer gets content updates. The last patch (10 October 2025) only closed a Unity security hole. It's discounted often, e.g. 75% off in the SEGA Publisher Sale of 13–27 August 2026. The "Full Health Collection" bundles the base game with every DLC.
Quick Start
- Earn 1 star to unlock the next hospital. You can move on right away and come back for 2 and 3 stars later. The later stars pay out big and count toward achievements (e.g. 45 gold stars for the base game).
- The patient pipeline is Reception → GP's Office → diagnosis rooms → treatment room → cured (or sent home). A missing or overloaded step creates queues everywhere.
- Build small at first. Cash flow early matters more than big rooms.
- Hire staff with the right qualifications. A Doctor for the GP's Office, Nurses for the Ward and Pharmacy, Assistants for Reception, and Janitors for maintenance.
- Train staff in a Training Room. Qualifications make them faster and let them cure more. Invest in this from year one.
- Keep people happy and comfortable: toilets, drink and snack machines, benches, radiators, plants, posters and windows all matter.
- Watch your cash. Loans are fine for expansion. Negative cash flow for long stretches isn't.
- Research new rooms and upgrades in the Research room to unlock cures and machine upgrades.
- Kudosh is earned from challenges and tasks and unlocks new items (decor, machines and more) across all hospitals.
Staff Types
Staff | Main jobs | Key qualifications (examples)
Doctor | GP's Office, diagnosis, Psychiatry, Surgery, Research | General Practice, Diagnostics, Psychiatry, Surgery, Research, Radiology
Nurse | Ward, Pharmacy, treatment rooms, injections | Treatment, Ward Management
Assistant | Reception, cafés, shops | Customer Service, Marketing
Janitor | Cleaning, machine maintenance, ghost capture, watering plants | Mechanics, Ghost Capture
Traits: staff have positive and negative traits (lazy, rude, messy, etc.). Check traits in the hiring pool before you pay a premium. Some negative traits are harmless for certain roles.
Training tips:
- Train your GP Doctors in General Practice. They see every patient and are the biggest bottleneck.
- Diagnostics speeds up diagnosis. Radiology and similar skills help in specific rooms.
- Achievements ask you to train a "World-Class" Researcher, Psychiatrist and Surgeon. World-Class is the top grade.
- Raise pay only when unhappiness about wages is the actual issue. Happiness also comes from staff rooms, decoration and reasonable workloads.
The Patient Pipeline
- Reception: staffed by an Assistant. Build a second desk in bigger hospitals.
- GP's Office: the first diagnosis. Build several, because they're always busy.
- Diagnosis rooms: General Diagnosis, Ward, Psychiatry, X-Ray, Cardiology, Fluid Analysis and others raise diagnosis certainty.
- Diagnosis threshold: in the hospital policy menu you can set the certainty patients need before treatment. A lower threshold means faster flow but more deaths and failed cures. Set it around the default to slightly lower if you have high-quality treatment rooms.
- Treatment: specialist rooms like the De-Lux Clinic (Light-headedness), the Pharmacy, Psychiatry (for mental conditions like Mock Star), the Ward and Surgery.
- Cured: each cure raises your reputation and the chance of better patients.
Queue management: in the room's info panel, check its queue length. If one room has 8+ in the queue, build a second copy of it before building anything else.
A First-Year Plan for Any New Hospital
- Read the star objectives first. They tell you which illnesses and rooms the level is built around.
- Build the core: Reception, two GP's Offices, a Pharmacy, toilets, a Staff Room and a drinks machine. That handles most early patients.
- Add General Diagnosis and a Ward next, then whichever specialist treatment room the incoming illnesses ask for.
- Open a Training Room early and send your GPs to General Practice training while the hospital is quiet.
- Add a Research room once cash flow is positive, and start on cure and machine upgrades.
- Check the illness list and queue sizes each month. Build for the illnesses you're actually seeing, not the ones you might see.
- Only then expand into extra plots. Buy land when your current building is full and profitable.
Money Management
- Treatment prices: you can set prices per treatment. Raise prices slowly on popular treatments while keeping an eye on patient happiness and demand.
- Marketing: run marketing campaigns (from the marketing menu; Assistants with the Marketing qualification help) to attract patients with specific illnesses you already treat well.
- Loans: take a loan to build a key room early, then pay it back quickly.
- Vending machines and later shops and cafés generate extra income and improve patient needs.
- Machine upgrades increase treatment efficiency. "Bells & Whistles" is an achievement for 25 machine upgrades.
Layout Tips
- Cluster related rooms: GP's Office and diagnosis rooms near Reception, and treatment rooms near diagnosis.
- Put waiting areas outside busy rooms with benches, heating and entertainment to keep patients content.
- Prestige matters: bigger, decorated rooms raise room prestige and staff happiness.
- Multiple buildings: buy extra plots to expand. Keep each building self-sufficient with its own toilets and vending machines.
- Corridors: keep main corridors at least 2 tiles wide so traffic doesn't jam.
Research, Epidemics and Events
- Research room: research new rooms, cures, machine upgrades and cure-rate improvements. Doctors with the Research qualification are faster.
- Superbug Projects: researchable tasks that share progress across players (e.g. "Hair Removal", "Learning Machine Learning", both tracked by achievements).
- Epidemics: infectious diseases spread. You'll get vaccines to inoculate patients, and containing an outbreak earns rewards.
- Ghosts: patients who die can come back as ghosts. Janitors with Ghost Capture handle them ("I. See. You." is an achievement for 100 captures).
- Year-end awards: an annual ceremony rewards hospital performance with cash and Kudosh.
Hospitals and Progression
The base game has 15 hospitals, each with 3 stars (45 gold stars total). Early hospitals include Hogsport, Lower Bullocks and Flottering, and later ones include Mitton University, Tumble and Flemington. Each expansion adds more hospitals. The gold-star achievements run from 15 up to 108 stars (in steps of 9 after 45), which lines up with three hospitals per expansion across all seven.
Illnesses you'll see include Light-headedness (a lightbulb head), Pandemic (a pan stuck on the head), Mock Star (musician impersonators), Animal Magnetism and Cubism. Each needs specific rooms or machines.

Common Mistakes to Avoid
- Too few GP's Offices. This is the number one bottleneck in almost every hospital.
- Hiring by cheapest salary. An unqualified, lazy doctor costs more in lost cures than a better one's wages.
- Skipping the Training Room. Qualifications compound for the whole game.
- Forgetting toilets and drinks. Patients leave or get sick in the corridor.
- Overbuilding early. Huge rooms you can't staff drain cash.
- Setting the diagnosis threshold too low. Deaths and failed treatments hurt your reputation.
- Ignoring machine maintenance. A broken machine can explode, killing the operator and wrecking the room.
Advanced Tips
- Specialize staff: give each doctor one role and train them toward it. Specialists heal faster than generalists.
- Price creep: in each hospital, bump high-demand treatment prices by small steps and check patient happiness.
- Staff room upgrades: decorate staff rooms heavily. Staff happiness keeps them productive and stops quitting.
- Challenges: accept the achievable ones. They pay cash and Kudosh, and some reduce costs.
- Run a Research doctor full-time: machine upgrades raise throughput across the whole hospital.
- Revisit early hospitals late: unlocked items and qualified staff make old 3-star goals easy.
