A co-op survival guide for Escape the Backrooms: how sanity, stamina and entities work, a level-by-level overview of what to do and what's hunting you, and tips for Hard/Nightmare runs and the new Level 421.

Researched September 2026 — current as of the 1.4.1 hotfix (7 September 2026), following the Level 421 "Plastic Mariana" update (27 August 2026). Live games change; check the linked sources if something differs.

Quick Start

  • Everyone has to reach the exit. A level only counts as escaped when every living teammate makes it out. No wanderer left behind.
  • Stay together. It's easy to lose each other in identical corridors, and some levels no-clip teammates into other areas. Move as a group and call out landmarks.
  • Entities can hear you. Proximity voice chat is in-game audio: shouting near an entity gets you found. Go quiet when you hear something.
  • Watch your sanity. Sanity drains over time and drops quickly when you're in danger. Almond Water restores it.
  • Manage stamina. Sprinting drains it fast. Save it for chases; Energy Bars restore it.
  • Learn each entity's rule. Every entity has its own mechanic (some hate light, some chase sound, some only move when unseen). Knowing the rule is how you survive it.
  • Hide when you can't outrun. Tables, closets, and closing doors break line of sight for many entities.
  • Read notes and look for codes. Many exits are puzzles: colour codes, symbol matches, switches, keys. The clue is usually somewhere on the level.
  • Save your flashlight, sprays and consumables for where they matter. Items are often level-specific tools.
  • Play with friends. It's playable solo, but it's designed for 2-4 players and cross-platform lobbies now make it easy to fill a group.

Core Mechanics

Sanity and stamina

Stat | Drains when | Restore with

Sanity | Over time; faster when in danger | Almond Water

Stamina | Sprinting, some actions | Energy Bars, resting

Speed boost | — | Juice

Stamina + speed | — | Moth Jelly (from Level 5's moths)

Low sanity causes hallucinations and effects that make navigation harder. Don't let it bottom out before a chase section.

Entities

  • Each entity follows a specific rule. Smilers lurk in the dark, Hounds can be stunned with flashlights, Death Moths and Partygoers each have their own behaviour, Kitty needs eye contact, and so on.
  • Sound is a weapon. Many entities home in on noise, including voice chat.
  • Line of sight matters. Some entities only chase what they see; break sight lines with doors and corners.

Co-op rules

  • Lobbies hold up to 4 players.
  • Voice is proximity-based.
  • If a teammate dies, the rest can still finish the level (depending on difficulty). On Nightmare there are no checkpoints or save files: if any player dies at any point, everyone restarts the whole playthrough.
  • Since the May 2026 update: cross-platform matchmaking via Epic Online Services (no Epic account needed), lobby codes for public and private lobbies, an option to hide the lobby code, and mute/kick/block/report tools.

Modes and difficulty

  • Story Mode runs the campaign levels; the Hub is the social/lobby space.
  • Difficulty: Normal, Hard and Nightmare. Hard and Nightmare are brutal; learn each level on Normal first.

Tip: exact codes and switch orders are often randomised between runs. Learn how each puzzle works rather than memorising solutions.

Level 421: Plastic Mariana (August 2026)

The first new level since 1.0 launched in October 2025.

  • Setting: an abandoned M.E.G. base conjoined with a children's hospital. You'll find Bruce's Discovery Center, then fall through a no-clip zone into the darkness of a giant ball pit.
  • New threats: two new entities and one entity variant. Assume something is hunting you in the balls; move with purpose and keep your team close.
  • Objectives (per the community Complete Escape Guide): activate the lighthouse lights to reveal the paths of white balls, find your way through, no-clip out, and toss plastic balls into basketball hoops.
  • Exit: it leads onward into Overgrowth.
  • 1.4.1 hotfix (7 September 2026): fixed crashes and lobby kicks when entering Level Fun+ after the 421 update.

Tips by Threat Type

Chasers (Smilers, Clump, Run For Your Life)

  • Know the route before you trigger the chase. Scout first when possible.
  • Don't sprint until you have to; start chases with full stamina.
  • Look for safe zones where a chase offers them (Level !~! has them); note what they look like on your first run.

Stealth entities (Partygoers, Death Moths, Skin Stealers)

  • Crouch and move slowly.
  • Stay behind cover and pause often to listen.
  • Don't talk in proximity chat when they're near.

Light-based entities (Hounds, Wretches, Animations)

  • Flashlights stun some entities; use them deliberately, not constantly.
  • In dark levels, mark walls with spray to avoid getting lost.

Eye-contact entities (Kitty, Windows)

  • Read the rule: some need you to watch them, others punish you for looking.
  • In co-op, assign one person to watch while others work.

Co-op Strategy

  • Assign roles: a navigator (map and landmarks), a puzzle solver, and a scout.
  • Split carefully: puzzles with multiple switches reward splitting up, but agree on a regroup point.
  • Share consumables: Almond Water for whoever is lowest on sanity.
  • Use lobby codes to keep random players out, and hide the code if you're streaming.
  • Cross-platform: PC (Steam), Xbox Series X/S, Xbox Game Pass, PS5 and Nintendo Switch 2 can all play together. Switch 2 needs Nintendo Switch Online for online play.

Hard and Nightmare Tips

  • Know every level on Normal first. You won't have time to explore on Nightmare.
  • Nightmare means one death resets the whole run (no checkpoints or saves). Play slower and safer, not faster.
  • Use consumables proactively before you're in trouble.
  • Practise chase routes repeatedly on Normal.
  • Stay quiet. Voice discipline matters even more on higher difficulties.

Common Mistakes to Avoid

  • Splitting up without a plan. Most wipes start with someone getting lost.
  • Talking loudly near entities. They can hear proximity voice.
  • Burning stamina on exploration. Save sprint for chases.
  • Ignoring sanity. Low sanity makes everything harder.
  • Assuming codes are fixed. Many are randomised; read the clues each run.
  • Running on the Level 8 bridge. It breaks; walk.
  • Playing Hard or Nightmare blind.

Advanced Tips

  • Learn entity audio cues. Each has a distinct sound; hearing it early gives you time.
  • Use doors aggressively. Closing doors behind you slows many pursuers.
  • Plan item routes: know which levels need a flashlight, spray or other key item, and don't waste them.
  • Unlock endings: some levels have more than one exit; different exits lead to different routes and endings. Try them all for achievements.
  • Report bugs on the developers' Feature Upvote page; the team has been responsive with patches.
