Back 4 Blood: Complete Guide

A co-op survival guide for Turtle Rock's Left 4 Dead successor: the card system, Cleaners, Ridden mutations, difficulty tiers, all six Acts, and the habits that keep a team alive on Nightmare and No Hope.

Researched September 2026 — current as of the final content patch (River of Blood, December 2022) plus the May 2024 Easy Anti-Cheat maintenance update. Content development ended in January 2023; servers and matchmaking are still up.

Status: Turtle Rock stopped developing new content in January 2023. The game is still sold on Steam and still gets occasional maintenance (the May 2024 update fixed Easy Anti-Cheat on Steam Deck), but expect no new balance changes. Everything below reflects the final state of the game.

Quick Start

  1. Build a deck before your first real run. Cards are the game. Unlock them in Fort Hope with Supply Points through the Supply Lines.
  2. Stay together. Most deaths come from being grabbed or pinned while alone.
  3. Don't shoot the birds or trip alarms. Crows, alarm doors and alarmed cars start hordes. Snitchers scream for a horde if they are shot badly or ignored.
  4. Spend copper every saferoom. Buy team upgrades, med supplies and attachments with what you pick up.
  5. Play Recruit, then Veteran. Nightmare and No Hope need good decks and Corruption knowledge.
  6. Ping everything. Enemies, ammo, items and hidden doors. Communication is half the difficulty curve.
  7. Keep a toolkit for Hazards. Many maps have locked supply doors, and toolkits open them.
  8. Solo is fine for learning. Offline solo with bots is available for practice.

The Card System

Before each run you pick a deck. As the run goes on you draw cards from it at each saferoom, and they persist for the rest of that run.

How to build a deck

  1. Order matters. Put your core identity cards first; you will have them from map one.
  2. Pick a role: damage, healer, tank or utility/support. A team with all four is easier than four damage decks.
  3. Cover your weaknesses. Include some stamina, ammo, health, or trauma mitigation depending on difficulty.
  4. Team cards stack. Several players bringing team-wide buffs (extra lives, more team health, ammo) makes the run easier for everyone.

Card economy notes

The Cleaners

Twelve Cleaners exist. The original eight come with the base game:

Cleaner Role tendency
Walker Precision damage
Holly Stamina and melee tank
Evangelo Mobility, escaping grabs
Hoffman Ammo support
Mom Team extra life support
Doc Healer
Jim Sniping and precision
Karlee Scouting and utility

The expansions added four more: Heng and Sharice (Tunnels of Terror), Prophet Dan (Children of the Worm) and Tala (River of Blood). Tala causes bleed damage to Ridden she attacks, can spawn Warped Chests, and puts a whistle on the map that summons Jeff, a friendly Tallboy who fights for the team.

Every Cleaner brings a personal perk and a team perk. Your team perks apply to everyone, so a full squad of different Cleaners is strongest.

The Ridden

Common infected

Basic Ridden come in hordes, and some variants are armoured or explode. Some Ridden can trap a Cleaner in a cocoon, so free cocooned teammates quickly.

Mutations (specials)

Mutations come in families. You will learn to identify them by sound:

Family Members What to do
Tallboy Tallboy, Crusher, Bruiser Shoot the weak spot on the arm; Crusher grabs, so free teammates quickly
Reeker Retch, Exploder, Reeker Kill them at range; don't let them pop in the middle of the team
Hocker Hocker, Stinger, Stalker Hockers pin with spit, Stingers are fast shooters, Stalkers pounce on isolated players
Snitcher Snitcher Kill it fast and cleanly or it calls a horde

Bosses

The expansions added more enemies, including the Slasher, Crone, Pusflinger and Sniper, plus Warped Ridden and Sentinel variants (achievements exist for killing each). Children of the Worm alone added four new enemy types.

Difficulty

Difficulty Notes
Recruit Learning; forgiving
Veteran Standard challenge; friendly fire and trauma matter more
Nightmare Needs a real deck and coordination
No Hope Highest tier; the Act 5 and Act 6 "Ace" achievements require it

Trauma is damage that lowers your maximum health for the rest of the map. Walking into a saferoom does not clear it; you need trauma-healing items, cards or the medical cabinets some saferooms offer. Higher difficulties inflict more trauma, which is why trauma-mitigation and healing cards matter on Nightmare and No Hope.

Campaign: Acts and Maps

The base campaign has four Acts; expansions added Act 5 and Act 6.

Every map has a secret. The achievements list names them, including "Pallet Cleanser" for The Dark Before the Dawn and "Easily Mist" for Blue Dog Hollow. Ridden Hives, from Tunnels of Terror, are optional underground areas with extra rewards.
