# Back 4 Blood: Complete Guide

A co-op survival guide for Turtle Rock's Left 4 Dead successor: the card system, Cleaners, Ridden mutations, difficulty tiers, all six Acts, and the habits that keep a team alive on Nightmare and No Hope.

*Researched September 2026 — current as of the final content patch (River of Blood, December 2022) plus the May 2024 Easy Anti-Cheat maintenance update. Content development ended in January 2023; servers and matchmaking are still up.*

> **Status:** Turtle Rock stopped developing new content in January 2023. The game is still sold on Steam and still gets occasional maintenance (the May 2024 update fixed Easy Anti-Cheat on Steam Deck), but expect no new balance changes. Everything below reflects the final state of the game.

## Quick Start

1. **Build a deck before your first real run.** Cards are the game. Unlock them in Fort Hope with Supply Points through the Supply Lines.
2. **Stay together.** Most deaths come from being grabbed or pinned while alone.
3. **Don't shoot the birds or trip alarms.** Crows, alarm doors and alarmed cars start hordes. Snitchers scream for a horde if they are shot badly or ignored.
4. **Spend copper every saferoom.** Buy team upgrades, med supplies and attachments with what you pick up.
5. **Play Recruit, then Veteran.** Nightmare and No Hope need good decks and Corruption knowledge.
6. **Ping everything.** Enemies, ammo, items and hidden doors. Communication is half the difficulty curve.
7. **Keep a toolkit for Hazards.** Many maps have locked supply doors, and toolkits open them.
8. **Solo is fine for learning.** Offline solo with bots is available for practice.

## The Card System

Before each run you pick a deck. As the run goes on you draw cards from it at each saferoom, and they persist for the rest of that run.

- **Player cards** are yours: stat boosts, weapon perks, team buffs and utility.
- **Corruption cards** are drawn by the game's Director each map: extra mutations, fog, hordes, bosses, mini-objectives and so on.
- A deck holds 15 cards. Your first cards come out in order, so the top of the deck is your early game.

### How to build a deck

1. **Order matters.** Put your core identity cards first; you will have them from map one.
2. **Pick a role:** damage, healer, tank or utility/support. A team with all four is easier than four damage decks.
3. **Cover your weaknesses.** Include some stamina, ammo, health, or trauma mitigation depending on difficulty.
4. **Team cards stack.** Several players bringing team-wide buffs (extra lives, more team health, ammo) makes the run easier for everyone.

### Card economy notes

- Supply Points come from playing. Spend them on Supply Lines in Fort Hope to unlock cards, cosmetics and more.
- Higher difficulties award more Supply Points and better loot.

## The Cleaners

Twelve Cleaners exist. The original eight come with the base game:

| Cleaner | Role tendency |
|---|---|
| Walker | Precision damage |
| Holly | Stamina and melee tank |
| Evangelo | Mobility, escaping grabs |
| Hoffman | Ammo support |
| Mom | Team extra life support |
| Doc | Healer |
| Jim | Sniping and precision |
| Karlee | Scouting and utility |

The expansions added four more: **Heng** and **Sharice** (Tunnels of Terror), **Prophet Dan** (Children of the Worm) and **Tala** (River of Blood). Tala causes bleed damage to Ridden she attacks, can spawn Warped Chests, and puts a whistle on the map that summons **Jeff**, a friendly Tallboy who fights for the team.

Every Cleaner brings a personal perk and a team perk. Your team perks apply to everyone, so a full squad of different Cleaners is strongest.

## The Ridden

### Common infected

Basic Ridden come in hordes, and some variants are armoured or explode. Some Ridden can trap a Cleaner in a cocoon, so free cocooned teammates quickly.

### Mutations (specials)

Mutations come in families. You will learn to identify them by sound:

| Family | Members | What to do |
|---|---|---|
| Tallboy | Tallboy, Crusher, Bruiser | Shoot the weak spot on the arm; Crusher grabs, so free teammates quickly |
| Reeker | Retch, Exploder, Reeker | Kill them at range; don't let them pop in the middle of the team |
| Hocker | Hocker, Stinger, Stalker | Hockers pin with spit, Stingers are fast shooters, Stalkers pounce on isolated players |
| Snitcher | Snitcher | Kill it fast and cleanly or it calls a horde |

### Bosses

- **Ogre**: the huge brute. Shoot the weak spots and don't let it corner the team.
- **Breaker** (and the armoured **Monstrous Breaker**): strip its armour and hit weak spots. Making it hurt itself is an achievement.
- **Hag**: a boss who swallows a Cleaner. Burst it down quickly.
- **Abomination**: the Act 4 finale.
- **The Harbinger**: a boss added by the expansions, fought in the chapter "Behold the Harbinger". The "Hard-Boiled" achievement asks you to beat it without the LAW, which tells you the LAW is the intended answer.

The expansions added more enemies, including the Slasher, Crone, Pusflinger and Sniper, plus Warped Ridden and Sentinel variants (achievements exist for killing each). Children of the Worm alone added four new enemy types.

## Difficulty

| Difficulty | Notes |
|---|---|
| Recruit | Learning; forgiving |
| Veteran | Standard challenge; friendly fire and trauma matter more |
| Nightmare | Needs a real deck and coordination |
| No Hope | Highest tier; the Act 5 and Act 6 "Ace" achievements require it |

Trauma is damage that lowers your maximum health for the rest of the map. Walking into a saferoom does not clear it; you need trauma-healing items, cards or the medical cabinets some saferooms offer. Higher difficulties inflict more trauma, which is why trauma-mitigation and healing cards matter on Nightmare and No Hope.

## Campaign: Acts and Maps

The base campaign has four Acts; expansions added Act 5 and Act 6.

- **Acts 1-3:** each Act is split into named chapters made of several maps. Chapter completions tracked by achievements include The Devil's Return, Search & Rescue, The Dark Before the Dawn, Blue Dog Hollow, The Armory, Plan B, Job 10:22, Dr. Rogers' Neighborhood and Remnants. The campaign opens on the map Resurgence.
- **Act 4:** a single finale against the Abomination.
- **Act 5 (Children of the Worm):** cultist-focused maps.
- **Act 6 (River of Blood):** five chapters travelling upriver by boat to find the cult and Tala's brother Derek. One of them is Blood Stream.

Every map has a secret. The achievements list names them, including "Pallet Cleanser" for The Dark Before the Dawn and "Easily Mist" for Blue Dog Hollow. Ridden Hives, from Tunnels of Terror, are optional underground areas with extra rewards.

### Other modes

- **Swarm:** PvP. Teams take turns as Cleaners and Ridden.
- **Trial of the Worm:** a free co-op mode where you pick maps and stack Corruption Modifiers for score, with leaderboards.
- **Solo:** play offline with bots.

## Weapons and Items

- Weapons come in colour-coded rarities, from common white up through green, blue and purple to gold Legendary. Higher rarities come with more and better attachments. Getting a kill with a Legendary is the "Overwhelming Power" achievement.
- Attachments matter more than the base weapon at low rarity; swap into better-attached guns as you find them.
- Melee attachments were added for all players in December 2022.
- Special weapons include the LAW, the Howitzer, Iron Claws, the Bow and joke items like Bob's Arm.
- Utility items: toolkits, pain meds, first aid, defibrillators, grenades, stun guns, Bait Jars, Skull Totems and food buffs (the "Balanced Meal" achievement wants five at once).

## Common Mistakes to Avoid

- **Running ahead.** Stalkers, Crushers and Hags punish lone players.
- **Shooting alarms and birds.** Watch for alarmed doors and crows on every map.
- **Hoarding copper.** Team upgrades bought early pay off for the whole run.
- **Bringing four greedy damage decks.** Someone needs healing and team survivability on high difficulties.
- **Ignoring Corruption cards.** Check the map's objectives at the start. Some pay out bonus rewards and some warn about a boss.
- **Wasting toolkits.** Save them for supply doors that contain team upgrades or strong gear.

## Advanced Tips

- **Deck order is strategy.** Front-load cards that fix early problems (stamina, ammo) and put late-scaling cards deeper.
- **Weak spots decide boss fights.** Every big mutation has glowing weak points. Precision builds melt them.
- **Melee builds are real.** With the right cards and a melee attachment, melee handles commons efficiently and saves ammo.
- **Stealth routes.** "Dead Quiet" asks you to finish a map without triggering hordes; practising it teaches you where every alarm is.
- **Trial of the Worm** is the best endgame loop for a skilled team: stack modifiers for bigger score multipliers.
- **Expansion content is shared.** A party can play River of Blood content as long as one player in the party owns it.

## FAQ

**Is Back 4 Blood dead?**
Content development ended in January 2023, but the game is still sold, servers work, and it received a maintenance update in May 2024. Matchmaking is quieter than at launch, so a friend group helps.

**Can I play solo?**
Yes. Solo offline with bots is available.

**Do I need the expansions?**
No. The base game has four Acts. The expansions add Acts 5 and 6, new Cleaners and new enemies. Only one player in a party needs to own an expansion.

**What is the hardest difficulty?**
No Hope.

**How do I unlock more cards?**
Earn Supply Points by playing, then spend them on the Supply Lines in Fort Hope.

**Is there PvP?**
Yes, Swarm mode.

**Does it work on Steam Deck?**
Yes. The May 2024 update fixed the Easy Anti-Cheat issue that blocked Steam Deck players.
